Most tours do Ephesus on day 1 (arrive via Izmir airport) then drive 3 hours to Pamukkale for sunset, visit the travertines day 2 morning, then onward.

About 180 km / 3 hours by road. There's no train — private transfer or tour bus.

Technically yes on a very long day (14+ hours), but it's exhausting and you'll rush both. Strongly recommend 2 days.

Both get busy 10am-3pm when cruise buses and tour groups arrive. Ephesus is worse if a cruise is in Kusadasi. Visit either at opening (8am) or late afternoon (after 3pm) for fewer crowds.

Highly recommended — most of what you're looking at is rubble without context. A good guide brings the 250,000-person Roman city to life. At Pamukkale, a guide is less essential but helpful for the Hierapolis ruins on the plateau.
